Before filling out the adoption application please read information regarding BARK’s adoption process here. If you do not receive a confirmation that your application has been submitted, do not worry.  It was submitted.

  • Please provide the name, address and telephone number of your current veterinarian. Please contact your vet and give his/her office permission to release information regarding your pet'(s) care to BARK.
  • Please provide the names, telephone numbers, and/or email addresses of two personal references (who are not relatives, roommates or your boyfriend/girlfriend)
  • Please add any information about yourself that you think would be useful in helping us select the right dog for you.
  • (Our current adoption fees are $250 for adult dogs. Elderly dogs or dogs with special needs may have a reduced fee. Rescue dogs are spayed/neutered, vaccinated, checked for heartworms and intestinal parasites and treated if necessary, on heartworm preventative, treated with flea/tick preventive, and microchipped).

I also give my consent for the transmittal or communication regarding my qualifications as a pet owner and veterinary history to BARK by any individual or institution listed above. I am also financially and physically able to care for an Australian Shepherd.
